How important is the documentation in the note? Extremely important! If it isn't documented, IT DIDN'T HAPPEN! As a provider, you may know that you ordered tests or reviewed labs, however, if you do not document it this can impact the level of your visit.

  • Have you incorporated all of the elements of the visit?
  • Chief complaint
  • History of present illness
  • Review of systems
  • PFSH (past medical, family, social history) areas
  • Exam
  • Assessment and plan
  • And the diagnosis
  • Have you listed the orders: labs, x-rays, etc.?
  • Have you signed and dated the medical note?

Let's talk about the first component of the medical visit: the chief complaint. What is a chief complaint? I often see "follow-up" as a chief complaint. "Follow-up" is NOT a chief complaint! You will not get credit for that, and remember, you must have a chief complaint to get credit for an Evaluation and Management visit. You CAN have "follow-up for hypertension" or "follow-up for sinus infection" but the follow-up must be clear.
